Priority repairs for safety and school security are needed. The broken window glass creates the potential for injuries. The wooden floorboards on the original ramps, decks and steps of both modular class buildings are very deteriorated. Holes are present on many of the ramp and deck floorboards creating trip and fall hazards for students and staff. The steps are deteriorated and loose also posing trip hazards. Some floorboards have deteriorated to a point where there is a concern of possible collapse under foot. Priority repairs for safety are needed. The floor tread covering is loose on the rear entry/exit ramp for modular building #R808. The metal trim on this ramp floor covering is loose and is coming up creating a trip and fall hazard. Priority repair for safety is required. Reattach loose door lock on the rear entrance of modular building #R808. Repair and seal hole in the wall of the art class kiln room #236. Repair ceiling and roof leaks in the following school locations: in the principal`s office, in the media center main room #140, in the main hallway outside and adjacent to the media center, in the main hallway at room #202A, inside of room #202A, in the hall adjacent to and inside of art classroom #238, in classroom #305, and in the custodial offices #218. 11. Cockroaches have been spotted in the principal`s office by staff. Professional pest control treatments are needed in this school building location. Repair loose wall panels on pre-k playground play structure. These loose wall panels slide sideways creating possible pinch hazards for children. There is also a loose panel on a small play structure on the 3rd through 5th grade playground that also creates a pinch hazard. This panel is missing a bolt in the bottom corner to secure it into place. Extra wood chips are needed to cover tree roots adjacent to picnic tables that are under the awning adjacent to the k-2 playground. These exposed tree roots create a trip and fall hazard adjacent to the metal picnic table. Reattach loose handhold/foot hold on climbing wall on the 3rd-5th grade playground. 4. cleaning needed on the walls and stalls in the girls and boys bathrooms on the 2nd grade hall rooms 225/226, in the bathrooms at room 155,in the 3rd grade pod boys and girls bathrooms, and in the 4th grade pod boys and girls bathrooms. Wallpaper needs replacing in certain areas of the girls/boys pod bathrooms. Few toilets need cleaning in the 3rd and 4th grade pod girls and boys bathrooms. There are cracks that are not easily cleanable in the boys/girls/staff restrooms in the 3rd and 4th grade pods. There are spots on the ceiling in the boy`s bathroom in the 4th grade pod that need to be investigated as a possible ceiling leak. Peeling paint is seen around the hand sinks in the girls 4th grade pod bathroom. 30.0

11. many classrooms, practice kitchen area for autistic children, custodial room, staff restrooms and other open areas have hand sanitizers, Clorox wipes, white board cleaner, cleaning chemicals, rubber cement, lotions, aerosol Lysol cans, bulk soap containers, all less than 5 ft. high where children can reach them. Most of these were corrected and moved as the inspection was done. Also all emergency red bags containing hand sanitizer also to be stored 5 ft. off the floor. These were also moved as the inspection was done. An email will be sent out as reminder as to how these products shall be stored. Couple of rooms had paint cans in them. These should be locked up in the janitor`s closet. The wooden walkways on the way to the 3rd and 4th grade pods are getting worn and need to be replaced at some point for safety reasons. 30.0
