Replace chipped and broken floor tile on the bathroom hallway. Repair roof leaks in both of the modular buildings /physical education activity trailers. Possible mold stains are present in the areas of the water damaged ceilings in the weight room trailer. Repair the sources of these roof leaks and replace the water stained and possible mold-stained ceiling panels in these trailers. The floor in the weight room trailer office at the exit door to the deck is extremely deteriorated. Priority repairs for safety are needed on this floor area due to the risk of possible collapse of the floor under foot. Railings are extremely deteriorated on both of the modular buildings rear exit decks with steps. These railings require priority safety repairs due to advanced deterioration. The railings are so loose that there is a serious risk of the rails collapsing under weight. This creates the potential of fall hazards. The railings are also very splintered and have exposed nail points which could cause potential injuries. Replace or remove loose anti slip treads on the entry ramp of the physical activity trailer for the wrestling program. These loose treads create the potential for trip and fall hazards.
Repair toilet that is nonfunctional due to leaks in the girl's hall bathroom in the school building. Repair chipped and damaged ceiling covering in the boy's bathroom in the church building. Reattach loose wall base tiles in both the boy's and girl's bathrooms in the school building. Toilet bowls in class bathrooms have interior buildup and residue and require regular cleaning.

Keep cleaning chemicals including Clorox surface wipes and Lysol aerosol spray cans stored in areas out of reach to students and accessible only to staff. Examples: Store cleaning chemicals in closed or locked cabinets, closets, or stock rooms. Remove class activity items from storage in classroom bathroom. Bathrooms are not clean storage locations. The class items stored in class restrooms can become a means of transfer for communicable disease pathogens including stomach flu. The storage of class items in class bathrooms is also an impediment to the cleaning and repair of these restroom facilities. Please remove.
Repair nonfunctional urinal in boys hall bathroom. This urinal has poor water flow. Clean and flush toilets weekly in unused classroom bathrooms. Some of these class bathroom toilets had build up inside due to lack of regular flushing and cleaning.
